These Terms of Service (“Terms”) are an agreement between you and Prosper Health & Aesthetics (“Prosper,” “we,” “us,” or “our”), a direct-pay medical wellness clinic operating at 5402 Morningside Ave, Sioux City, IA 51106. They govern your access to and use of https://www.prosperdirecthealth.com, our online shop, and any related services provided by our clinic.

A patient-provider relationship is established only after you complete an intake, are evaluated by a Prosper Health provider, and a treatment plan is agreed upon in writing.
3. Services and Results
Cosmetic, wellness, and medical services offered by Prosper involve inherent medical risk. Results vary from person to person and depend on factors outside our control, including genetics, health history, lifestyle, and treatment compliance. We make no guarantee or warranty, express or implied, about the outcome of any service. Your provider will discuss realistic expectations, risks, and alternatives with you before any treatment begins, and you will be asked to sign treatment-specific informed consent.
4. Booking, Cancellations & No-Shows
Appointments can be booked online, by phone, or in person. We request at least 24 hours notice for cancellations or reschedules. No-shows or cancellations with less than 24 hours notice may be subject to a cancellation fee at the discretion of the clinic. Treatments requiring a deposit (e.g., memberships, certain aesthetic packages) are governed by the specific terms provided at time of booking.
5. Online Shop
Products sold through our online shop are curated by the Prosper clinical team and intended for personal use. Orders are final when payment is accepted. Prices are listed in US dollars and may change without notice. Applicable sales tax is calculated at checkout when required.
Payments are processed by Stripe, a PCI-compliant third-party payment processor. We do not store your full card details on our servers.

6. Dietary Supplements Disclaimer
Dietary supplements sold through our shop are not drugs and have not been evaluated by the U.S. Food and Drug Administration (FDA). These Terms are governed by the laws of the State of Iowa without regard to its conflict-of-laws principles. Any legal action arising out of or related to these Terms will be brought exclusively in the state or federal courts located in Woodbury County, Iowa, and you consent to the jurisdiction of those courts.

What you’ll receive: If you provide your mobile number and check the SMS opt-in box on a https://www.prosperdirecthealth.com form (including our contact form) or opt in at the clinic, you will receive recurring automated text messages from Prosper Health & Aesthetics related to your appointments, including: appointment booking confirmations, 24-hour appointment reminders, day-of arrival and prep instructions, reschedule or cancellation notices, and appointment follow-ups.
Message frequency: Frequency varies based on your appointment activity, typically 1–4 messages per scheduled appointment.
Message and data rates:Msg&data rates may apply. Your wireless carrier’s standard rates apply to any messages sent or received. Carriers are not liable for delayed or undelivered messages.
How to opt out: Reply STOP to any message from us at any time to unsubscribe from the SMS program. You will receive one final confirmation of your opt-out.

Consent is not a condition of purchase. You can book and receive any service from Prosper Health without consenting to SMS messages. Opting out of SMS will not affect your care or your ability to schedule appointments by phone, email, or in person.
